目的 探讨心脏恶性肿瘤的心肌超声造影(MCE)表现。方法 回顾性分析27例心脏恶性肿瘤的常规经胸超声心动图(TTE)、MCE及临床病理资料,根据病理及临床诊断结果分为原发肿瘤组6例和继发肿瘤组21例,计算肿瘤A值与正常心肌A值的比值(A值:超声造影剂峰值强度),比较其TTE、MCE表现的差异。结果 27例心脏恶性肿瘤患者中,MCE呈高增强19例、低增强8例。其中原发肿瘤组6例均呈高增强,继发肿瘤组21例呈高增强、低增强的分别为13例、8例,两组增强程度比较差异无统计学意义(P=0.136);原发肿瘤组与继发肿瘤组的肿瘤A/心肌A值分别为2.31±0.65、1.25±0.51,差异有统计学意义(P<0.05)。肿瘤直径≤5.0 cm时呈高增强、低增强分别为3例、6例,肿瘤直径>5.0 cm时呈高增强、低增强分别为16例、2例,两者增强程度比较差异有统计学意义(P=0.006)。结论 本研究心脏原发恶性肿瘤MCE均呈高增强,心脏继发恶性肿瘤MCE呈高增强或低增强,直径>5.0cm的心脏恶性肿瘤MCE大多数呈高增强。 |

Preliminary Discussion on Myocardial Contrast Echocardiography of Cardiac Malignant Tumor |

Abstract: |
Objective To discuss the imaging features on myocardial contrast echocardiography of cardiac malignant tumor. Methods Twenty-seven patients with cardiac malignant tumor were evaluated and the routine transthoracic echocardiography(TTE),myocardial contrast echocardiography(MCE) and pathological characteristics of cardiac malignant tumor were analyzed. Twenty-seven patients with cardiac malignant tumor were divided into primary tumor group(6 cases) and secondary tumor group(21 cases) according to the pathological and clinical diagnosis results. The findings of TTE, MCE and ratio of tumor A value to normal cardiac A value (A value:peak intensity of ultrasound contrast agent) were compared between primary tumor group and secondary tumor group. Results 6 primary cardiac malignant tumors (100%) were all highly enhanced,13 secondary cardiac malignant tumors(61.90%) were highly enhanced while 8 secondary cardiac malignant tumor(38.10%)were lowly enhanced. There were no differences in the enhancement degree between primary and secondary cardiac malignant tumor(P=0.136). Tumor A/ myocardium A of primary and secondary cardiac malignant tumor were respectively 2.31±0.65,1.25±0.51. There were significant differences in tumor A/ myocardium A of MCE between primary and secondary cardiac malignant tumor(P<0.05). When tumors were≤5 cm,high and low enhancement were illustrated in 3,6 cases,respectively. When tumors were>5 cm,high and low enhancement were illustrated in 16,2 cases,respectively. There were significant differences in enhancement degree between lesion size of ≤5 cm and >5cm on MCE(P<0.05). Conclusion The MCE of all cardiac primary malignant tumor was highly enhanced and the cardiac secondary malignant tumor was highly or lowly enhanced. Most of the cardiac malignant tumors with > 5.0cm in diameter were highly enhanced in MEC. |
